Place the Concord Grape Jelly in a medium, microwave safe bowl and warm at 30 second intervals in the microwave until thinned. Whisk until smooth and add in 2 cups of yogurt. Whisk well and set aside.
Place the grape yogurt mixture into a measuring cup with a pourable spout. Fill each popsicle mold 2/3 of the way full. Place in the freezer for 20 minutes to allow the surface to start to freeze for better separation.
Remove from freezer and top each popsicle with a thin layer of plain yogurt. Insert popsicle sticks and freeze for a minimum of 24 hours before serving.
